I (we) understand that the information contained herein shall be kept completely confidential and used by the Stark County Out of Partnership, Inc. (SCOPP) and the City of Canton for the purpose of determination my (our) eligibility for participation in he First Time Homebuyer Down Payment Assistance Program.  I (We) further grant SCOPP and the City of Canton the right to release this information to any lending institution in the program.

I (We) certify that I (we) am (are) aware that the occupants of the above indentified property, and the information contained herein is true, accurate and complete.

I (We) grant our permission to SCOPP to obtain information on my (our) current and past credit and borrowing history.
